We now have a clear picture of what the top five job roles reporting to CIRAS were in 2025/26. Train crew made up the largest share. This follows the trend of the previous two years. The year 2025/26 also saw a considerable increase in train crew raising concerns with CIRAS. Operations management roles are among the top five. This includes managers responsible for the planning or running of day-to-day operations, such as heads of safety, technical leads or consultants.
Rules and procedure continues to be our most reported on theme so far this financial year. The theme of health and wellbeing is close behind. Reports of this type discussed concerns about psychological safety at work or the limitations of work wellbeing policies.
As in previous years, there continued to be an approximate 40%/30% split between the perceived root causes 'risk management' and 'infrastructure, vehicles, equipment and clothing' in 2025/26. Risk management is about companies either not identifying a safety hazard or rectifying a known safety hazard in time.
